1. Agreement to Terms

By accessing or using the Teddy Meds platform at teddymeds.com (the "Platform"), you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree with any of these terms, do not use the Platform.

2. Nature of Services

Teddy Meds is a telehealth technology platform that facilitates connections between patients and independent, licensed healthcare providers. Teddy Meds does not practice medicine, provide medical advice, or employ the healthcare providers on its network. All clinical and prescribing decisions are made solely by the independent, licensed provider assigned to your case.

3. Eligibility

To use our services, you must:

• Be 18 years of age or older
• Reside in a state where Teddy Meds is licensed to offer services at the time of your evaluation
• Provide accurate, complete, and current health and personal information
• Not use the Platform for any unlawful purpose

4. No Guarantee of Treatment

Paying a consultation fee or completing an intake form does not guarantee that a prescription will be issued or that you will receive treatment. All clinical decisions are at the sole discretion of your assigned licensed provider. If you are determined ineligible following clinical review, your consultation fee will be refunded in full.

5. Medical Disclaimer

The content on this Platform is provided for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult a qualified healthcare provider for questions about your specific health condition. In a medical emergency, call 911 immediately.

6. User Responsibilities

You agree to:

• Provide accurate, truthful health and personal information
• Use medications only as prescribed by your provider
• Report adverse effects promptly to your provider
• Not share your account or medications with others
• Not use the Platform to obtain medications for non-medical or recreational purposes

7. Privacy

Your use of the Platform is also governed by our Privacy Policy and HIPAA Notice of Privacy Practices, both of which are incorporated by reference into these Terms.

8. Refund Policy

If you are determined ineligible for treatment following a provider evaluation, your consultation fee will be refunded in full. Any charges for medications already shipped or dispensed are non-refundable. Refund eligibility for subscription plans, if offered, will be disclosed at the time of purchase. 9. Intellectual Property

All content on the Platform, including text, graphics, logos, and software, is the property of Teddy Meds and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written permission.

10. Limitation of Liability

To the maximum extent permitted by applicable law, Teddy Meds and its affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Platform or any healthcare services facilitated through it. Our total liability for any claim shall not exceed the amount you paid to us in the 12 months preceding the claim.

11. Compounded Medications

Medications dispensed through this Platform may include compounded preparations prepared by DEA-registered, 503A-certified compounding pharmacies. Compounded medications are not FDA-approved products and have not been evaluated by the FDA for safety, efficacy, or quality. They are prepared based on a valid, individualized prescription from a licensed healthcare provider.

12. Governing Law
