Telehealth Standards

Telehealth Informed Consent

Notice: This document is presented to patients prior to receiving any medical evaluation or treatment through the Teddy Meds platform. By proceeding with your intake, you acknowledge and agree to the terms described below.

1. Nature of Telehealth Services

Teddy Meds operates as a telehealth technology platform that facilitates connections between patients and independent, licensed healthcare providers. Care is delivered through a two-step process:

  1. Health assessment. You complete a detailed questionnaire that captures your medical history, current medications, allergies, and goals. No clinical or prescribing decision is made at this stage.
  2. Live video consultation.A New York–licensed physician conducts a real-time, synchronous video visit with you before any prescription is issued. During this visit the physician reviews your assessment, asks follow-up questions, verifies your identity, and determines whether treatment is clinically appropriate.

This synchronous video consultation satisfies the live, real-time telehealth requirements of New York Public Health Law Article 29-G § 2999-cc. No prescription is issued without a completed live video visit.

Teddy Meds is not a medical practice and does not practice medicine. All clinical decisions are made solely by the independent, licensed physician assigned to your case.

1A. Identity Verification

Before any prescription can be issued, you must present a valid government-issued photo identification (driver’s license, passport, or state-issued ID) during your live video consultation. The physician visually confirms that the identification matches the name, date of birth, and information provided in your intake. If identity cannot be verified, the consultation will be discontinued and no prescription will be issued.

2. My Rights As a Patient

As a patient using telehealth services through this platform, you have the right to:

  • Know the name, professional credentials, and contact information of the provider evaluating your case
  • Receive a complete explanation of any proposed treatment, including risks, benefits, and alternatives
  • Refuse any proposed treatment without affecting your access to other services
  • Have your health information kept private in accordance with HIPAA and our Privacy Policy
  • Seek a second opinion from an independent provider of your choosing
  • Contact your provider with questions or concerns during your treatment period
  • Discontinue treatment at any time

3. No Guarantee of Prescription or Treatment

Completing the intake questionnaire and a live video consultation does not guarantee that a prescription will be issued or that you will be accepted for treatment. All treatment decisions rest entirely with the licensed physician assigned to your case.

Payment is collected only after the physician has reviewed your assessment, completed the live video visit, and determined that a prescription is clinically appropriate. If you are determined to be ineligible, no charge is made.

4. Risks of Telehealth

Telehealth has benefits and limitations compared to in-person care. You acknowledge that:

  • A physical examination is not performed as part of this telehealth evaluation
  • Lab results submitted are reviewed by the provider but were not ordered as part of this visit
  • Technical failures, internet connectivity issues, or platform outages may affect service delivery
  • Telehealth may not be appropriate for all conditions; acute or emergency conditions require in-person care
  • If you experience a medical emergency, call 911 or go to your nearest emergency room immediately

5. Prescription Medications

Any medications prescribed through this platform are issued at the sole, independent discretion of a licensed provider. All medications are dispensed from DEA-registered, 503B-certified compounding pharmacies or sourced from licensed US manufacturers. Compounded medications are not FDA-approved products.

You agree to use all medications only as prescribed and to promptly report any adverse effects to your provider or, in case of an emergency, call 911.

6. State Availability

Teddy Meds currently offers telehealth services only to residents of the State of New York. Services are not available in any other state. You confirm that you are physically located in New York at the time of your evaluation and video consultation.

7. Contact Your Provider

After your provider has reviewed your intake and issued a prescription (if applicable), you may reach your care team through the contact methods provided to you at the time of your evaluation. You may also contact our support team at contact@teddymeds.com or 888-990-7127.

8. Acknowledgment

By checking the consent box during your intake and submitting your health information, you confirm that:

  1. You have read and understood this Telehealth Informed Consent
  2. You agree to the Terms of Service and Privacy Policy
  3. You understand that a prescription is not guaranteed and depends on provider evaluation
  4. You consent to receiving telehealth services from licensed providers
  5. The information you have provided in your intake form is accurate and complete to the best of your knowledge

Compounded medications are prepared by Olympia Pharmaceuticals, a DEA-registered, 503B-certified compounding pharmacy compliant with USP <795> and <797> standards. Compounded medications are not FDA-approved products. Prescriptions are issued solely at the discretion of licensed healthcare providers. Services are available exclusively to residents of New York State.
