1. Nature of Telehealth Services
Teddy Meds operates as a telehealth technology platform that facilitates connections between patients and independent, licensed healthcare providers. Medical care is provided via asynchronous store-and-forward telemedicine: you complete a detailed health questionnaire, and a licensed provider independently reviews your information and makes prescribing decisions.
Teddy Meds is not a medical practice and does not practice medicine. All clinical decisions are made solely by independent, licensed healthcare providers.
2. My Rights As a Patient
As a patient using telehealth services through this platform, you have the right to:
- Know the name, professional credentials, and contact information of the provider evaluating your case
- Receive a complete explanation of any proposed treatment, including risks, benefits, and alternatives
- Refuse any proposed treatment without affecting your access to other services
- Have your health information kept private in accordance with HIPAA and our Privacy Policy
- Seek a second opinion from an independent provider of your choosing
- Contact your provider with questions or concerns during your treatment period
- Discontinue treatment at any time
3. No Guarantee of Prescription or Treatment
Completing the intake questionnaire and paying a consultation fee does not guarantee that a prescription will be issued or that you will be accepted for treatment. All treatment decisions rest entirely with the licensed healthcare provider assigned to your case.
If you are determined to be ineligible for treatment following clinical review, you will be notified and a refund of your consultation fee will be processed in full.
4. Risks of Telehealth
Telehealth has benefits and limitations compared to in-person care. You acknowledge that:
- A physical examination is not performed as part of this telehealth evaluation
- Lab results submitted are reviewed by the provider but were not ordered as part of this visit
- Technical failures, internet connectivity issues, or platform outages may affect service delivery
- Telehealth may not be appropriate for all conditions; acute or emergency conditions require in-person care
- If you experience a medical emergency, call 911 or go to your nearest emergency room immediately
5. Prescription Medications
Any medications prescribed through this platform are issued at the sole, independent discretion of a licensed provider. All medications are dispensed from DEA-registered, 503A-certified compounding pharmacies or sourced from licensed US manufacturers. Compounded medications are not FDA-approved products.
You agree to use all medications only as prescribed and to promptly report any adverse effects to your provider or, in case of an emergency, call 911.
6. State Availability
Telehealth services are available only in states where Teddy Meds and its provider network hold appropriate licenses. Services are not available in states where we are not licensed. You confirm that you are located in an eligible state at the time of your evaluation.

By checking the consent box during your intake and submitting your health information, you confirm that:

- You consent to receiving telehealth services from licensed providers
- The information you have provided in your intake form is accurate and complete to the best of your knowledge
